New Patient Registration

Come into surgery to pick up or form us register using this link https://gp-registration.nhs.uk/P81010 

New patients need to be assessed as to whether they are within the catchment area of the practice before being accepted. You will need your NHS card and the name of your old GP to register.

You will be issued with a computer number, which is a unique number we use to identify records pertaining to each patient. Please try and remember this as it vastly speeds up processes such as booking appointments and repeat prescriptions.

Your named GP is Dr A Robinson. 

    

 

Please note this is just for information purposes and you can make appointments with any GP of your choice.

Medical treatment is available from the date of registration. Please contact reception for further information.

Leaving The Practice

If you leave the area please let the surgery know as soon as possible. You will be expected to register with a new GP in that area.
